You can Update your contact details online when they change. You can also download and complete paper form and bring it along with your proof of ID to the surgery. Paper forms are still available at reception.
Cancel a future appointment at the surgery anytime. You will receive a reminder the day before which has a link allowing you can cancel an appointment but this link lets you cancel at any time. You can still call the surgery at any time during working hours to cancel an appointment.
The surgery can provide for you Travel Vaccines. We have Information about travel the vaccines available and their prices as well as an online request form. Paper forms cab be downloaded and are available from reception. Download a paper copy of the form.
Give us your feedback by completing our Friends and Family pack. We may periodically ask you for feedback via a text message. All responses and feedback are very much appreciated and we will action all issues raised. Download a paper copy of the form.
Should you require a FIT/Sick Note or the renewal of a FIT/Sick note then you can request it online. Again paper forms can be downloaded and are available from reception.
You can provide feedback using our Friends and Family link but if you want to make a more formal complaint please use this link to submit online or access the paper form.
If you are monitoring your Blood Pressure (BP) at home you can use one of our Blood Pressure Diaries which are available in either Excel or Word format. Alternatively you can fill out our online form.
Request Online access to you records via the NHS App. Create an account, change an existing account or request proxy services (where you grant permission for others to view your medical record)
The surgery uses messaging services to keep in contact with patients about their health. If you use the NHS App or Patient Access then these messages will appear in your app. If you don’t use these tools you will receive them as text message on your mobile phone.
We will contact you to provide information about your conditions or recent tests or to invite you to book appointments for things such as health checks, tests or vaccination programmes (for example Flu, COVID, RSV etc.) directly.
Patients with Asthma or COPD may be asked by their GP to keep a diary of their Peak Flow Readings.
A guide to Peak Flow testing and how to correctly use a Peak Flow Meter can be found at Asthma and Lung UK. This guide also includes how to maintain a diary. We have a Microsoft Excel diary as well as a paper diary for you to complete. Once complete the diary can be emailed to the surgery using [email protected].
